What is The value of Doppler ultrasound in the evaluation of the severity of liver disease in patients with primary biliary cirrhosis (PBC) about?

Purpose: To correlate SMA volume flow (VF) with inflammatory bowel disease (IBD) activity. Materials and Methods: Doppler sonography of the SMA in 69 fasted patients (45 Crohns, 4 ulcerative colitis, 14 controls) was performed with a 2-4 MHz probe on the ATL Ultramark 9. Peak systolic, minimum diastolic, time averaged velocity, RI and vessel diameter were measured. Volume flow (VF) was measured using inbuilt software using the formula yd2 v. Three readings averaged. Error minimised by keeping Doppler angle B60°, gate spanning vessel and maximum zoom to measure vessel diameter. Doppler indices were correlated with the Crohns disease activity index (CDAI), C-reactive protein and Barium studies. VF \500 ml min cut-off for active disease. Results: All patients with VF\500 ml had a raised CDAI (\ 150). CRP correlated poorly with CDAI and VF, but VF correlated well with barium findings. In six patients followed sequentially, there was a good correlation between CDAI and VF. Conclusion: VF\ 500 ml min only found in active CD. Take home points: SMA VF \ 500 ml min=active CD. VF provides additional information to CDAI and CRP. VF is a simple way of monitoring CD activity.
