David, a 52-year-old man, presented with shortness of breath, cough, wheezing, and sinus symptoms. Tests showed pulmonary infiltrates, eosinophilia, and hypokinesis. A biopsy found eosinophilic myocarditis. Treatment with steroids significantly improved his symptoms and follow up biopsy showed the myocarditis had resolved. He was ultimately diagnosed with eosinophilic granulomatosis with polyangiitis (EGPA), a rare vasculitis characterized by asthma, sinusitis, eosinophilia, and organ inflammation.
