What is In Response to Dr. Karger et al. about?

To the Editor: Driven by the imperative warning in its title, our attention was raised to the theoretical study of Das xu et al. (1). To put their results in right perspective to heavy ion radiotherapy, we feel that some comments are necessary. The authors state that the clinically relevant relative biologic effectiveness (RBE) increases with dose per fraction, which on the first view seems to be in contradiction to available experimental data. This finding, however, is a direct result of their particular RBE definition, which compares isoeffective doses of potentially different fractionation regimes (Fig. ). Of course, when comparing clinical data from low-and high-linear energy transfer (LET) treatments, dose per fraction must be (and actually is) considered (2). Including fractionation effects into the RBE, however, is confusing because the term RBE is used to characterize solely the different biologic effectiveness of high-relative to low-LET radiation under otherwise identical conditions. Therefore separate factors W RBE and W a/b should be used (3), which may be combined to an overall weighting factor W IsoE = W RBE $ W a/b . For two low-LET regimens with different dose per f
