Late cutaneous hypersensitivity reaction: case report A 31-year-old man developed late cutaneous hypersensitivity reaction following ingestion of tonic water containing quinine. The man had ingested tonic water containing quinine [dosage not stated]. Subsequently, he presented with a tonic-caucustic syndrome consisting of aphthous ulcers on the lips and in the oral cavity. After 24 hours, he developed a fixed erythematous lesion on the second phalanx of the right hand. His clinical examination was normal. Previously, he ingested gin [alcohol].The man discontinued quinine, and his symptoms resolved. Using prick tests screening for sensitisation to airborne allergens and routine foods was performed, which revealed negative result. The patch test was positive for quinine. The prick test for chloroquine was negative. He underwent provocation with quinine at home, which revealed positive result and again presented with the same symptoms. Based on these findings and clinical presentation, it was concluded that he developed late cutaneous hypersensitivity reaction associated with quinine.
