This case presentation describes an older female who presented to the emergency room with double vision and a left arm tremor. On examination, she was found to have a right pupil that was dilated and nonreactive, left eye lateral strabismus, and involuntary movements and weakness on her left side. She was diagnosed with Benedikt syndrome, which is caused by a stroke in the midbrain tegmentum and is characterized by an ipsilateral oculomotor nerve palsy and contralateral involuntary movements and hemiplegia.
