What is Natriuretic Peptides about?

D espite years of clinical investigation, there has been a paucity of efficacious treatment for heart failure with preserved ejection fraction (HFpEF) until recently. Consistent observations from randomized controlled clinical trials support the crucial role of sodium glucose cotransporter 2 (SGLT2) inhibition in reducing heart failure morbidity in patients with or without diabetes across the spectrum of left ventricular function. Data from clinical trials show, in comparison with placebo, that SGLT2 inhibition lowers the relative risk of worsening heart failure or cardiovascular death by approximately 20% to 25% over a 16-to 36-month period. A number of candidate mechanisms may explain these benefits. Chief among them is the role of SGLT2 inhibition on priming the natriuretic and metabolic reserve of the kidney. Since their clinical utility was first demonstrated, B-type natriuretic peptide (BNP) and its biologically inert split product amino terminal pro-BNP (N-terminal pro-B-type natriuretic peptide [NT-proBNP]) have been critical tools for the diagnosis and prognosis of patients with heart failure.
