An 8-year-old girl fractured her upper right permanent central incisor tooth after slipping and hitting her mouth. On examination, the tooth appears to be intruded rather than fractured. A periapical radiograph shows the tooth is severely intruded with an immature root and open apex. There are also small radiolucencies on the crown that are likely areas of enamel hypoplasia or hypomineralization.
