(1) A 55-year old HIV-positive man with a history of non-compliance to antiretroviral therapy presented with fever, diarrhea, weight loss and generalized weakness. (2) Chest imaging showed a cavitary lesion in his left upper lobe containing a soft tissue mass compatible with aspergilloma (fungal ball). (3) Aspergilloma is a non-invasive form of pulmonary aspergillosis that occurs in pre-existing lung cavities. It was diagnosed based on imaging findings and clinical history.
