What is Catheter Ablation Versus Anti-arrhythmic Therapy For First Line Treatment Of Atrial Fibrillation: A Systematic Review And Meta-analysis Of Randomized Controlled Trials about?

Background: Increasing atrial fibrillation (Afib) is leading to stroke-related mortality and economic burden. Multiple studies have found catheter ablation to provide longer periods of rhythm control than antiarrhythmic drugs (AAD). Current guidelines reserve it for low risk patients or those failing AAD use. Building upon prior meta-analyses, we wanted to investigate if Afib recurrence was lower in patients initially treated with catheter ablation as compared to those treated with AAD only. Methods: Systematic literature review with keywords "catheter ablation" and "atrial fibrillation" was conducted for randomized controlled trials between 2000 to 2022. PubMed, Scopus, and Cochrane Library were searched using PRISMA guidelines. Studies were excluded if they did not report a cohort randomized to first-line catheter ablation. A meta-analysis of 7 chosen studies was done for the relative odds of Afib recurrence using the Mantel-Haenszel method. Results: Seven studies involving 1778 patients were included. The ablation group had less recurrent Afib with 327 of 936 patients [34.93%] as compared to the AAD group with 493 of 842 [58.55%]. The odds of recurrence were significantly lower
