Kyle Cherrie C. Matoza lost her Philippine passport and U.S. visa during a family trip to Boracay in June 2022. She had brought her passport as valid identification for the trip. Upon returning to Manila, the envelope containing the passports was missing. Despite efforts to locate it, the passport could not be found. Matoza is filing this affidavit of loss in order to apply for a new Philippine passport and new U.S. visa to replace the lost documents.
