What is Acute Myeloid Leukaemia with Inv(3)(q21.3q26.2) about?
A 25-year-old man presented with fatigue and bruising. His full blood count showed Hb 70 g/l, MCV 105 fl, WBC 0.5 × 10 9 /l, neutrophils 0.1 × 10 9 /l and platelets 200 × 10 9 /l. His blood film (not illustrated) showed small numbers of myeloblasts. The bone marrow aspirate was hypercellular with 60% myeloblasts (CD34+, CD117+, CD41-, CD61-) showing prominent nucleoli and minimal differentiation (top left) (all images ×100 objective). Notably, there was also a large population of abnormal megakaryocytes, including micromegakaryocytes, some binucleated or with non-lobated or bilobed nuclei, many of which were actively shedding platelets (top and bottom left). Note the myeloblast population showing no differentiation and prominent nucleoli. A micromegakaryocyte should have a non-lobated nucleus and be of a similar size to a promyelocyte (bottom left image is typical). Metaphase cytogenetic analysis showed inv(3)(q21.3q26.2). The patient sadly failed multiple lines of induction chemotherapy and each phase of chemotherapy-related cytopenia was followed by a rapid early rebound (10 days) in the platelet count. Normally platelet recovery suggests clearance of an acute leukaemia following
