Olanzapine misuse Hyponatremia, rhabdomyolysis and inappropriate secretion of antidiuretic hormone: case report A 57-year-old man (initials: MR) developed hyponatraemia, rhabdomyolysis and inappropriate secretion of antidiuretic hormone (ISADH) following intentional misuse of olanzapine for schizophrenia. The man was admitted to the ICU of the emergency room for apyretic loss of consciousness. It was reported that he had been receiving treatment with olanzapine 10 mg/day [route not stated] for schizophrenia. Due to poorly controlled symptoms, he had increased the daily dosage of olanzapine by himself (intentional misuse and treatment non-compliance). On the day of admission, his family found him unconscious, which prompted a visit to the emergency room. On admission, his Glasgow Coma Scale score was 6, and examination showed pupils were reactive and equal, with no extrapyramidal syndrome or neurological deficits and no signs of seizures. It was concluded that he was in coma. He was polypneic at 34 cpm, on room air his oxygen saturation was 80% with paradoxical breathing and respiratory pauses. His HR was 110 bpm, BP was 115/70mm Hg and capillary refill time <3s. Capillary glycaemia
