What is Delayed sleep-wake phase disorder (DSWPD) and salivary melatonin secretion impairment in a patient with a pineal cyst about?

We present the case of a 19-years old man with a non-compressive pineal cyst suffering from a delayed sleep-wake cycle. A baseline polysomnographic examination (PSG) with salivary melatonin sampling and body temperature measurements were performed. The melatonin levels were slightly elevated in the morning, and body temperature curve displayed a normal reduction during sleep. After 7 months concomitant treatment with progressive exogenous melatonin administration up to 6 mg, 5 h prior to habitual sleep onset (HSO) and 30 min of bright light therapy after waking up, the sleep-wake cycle was advanced and stabilized by 3 h. Both subjective well-being as well as different psychometric values such as insomnia severity and sleep quality significantly improved compared to baseline. The relationship between anatomic lesions in the pineal region, i.e. pineal cysts, and melatonin secretion remains controversial, and few studies have been published on this behalf. Nevertheless, the stable advance in sleep-wake cycle of our patient, after the aforementioned treatment, encourages further research.
