In a retrospective case series of 11 patients, 6 women aged 60-74 years were described, who exhibited lack of efficacy during treatment with amitriptyline, clonazepam, gabapentin, ibuprofen, nortriptyline, paracetamol, duloxetine or pregabalin for oral dysesthesia, persistent idiopathic facial pain (PIFP) or post-traumatic trigeminal neuropathy [PTTN; not all rotes and dosages stated]. Case 1: A 69-year-old woman presented with a 3 month history of symptoms of constant dry mouth and a burning/particulate sensation affecting her palate, lower labial mucosa and tongue, with pain up to 7/10 intensity. Upon presentation, she was diagnosed with an oral dysesthesia. Sequential trials of topical clonazepam 0.1 mg/mL solution 5 minute swish and spit twice daily, gabapentin 300mg 3 times daily and amitriptyline 25mg nightly were ineffective. Thereafter, at a follow up visit, she reported relief of her symptoms when she would be chewing gum and press the gum against the anterior palate. A trial of orthodontic wax placed against the anterior palate relieved her symptoms. Also, an acrylic palatal appliance provided significant reduction in her symptoms. She was subsequently lost to follow-up.
