RATIONALE: A label of antibiotic allergy has important implications on future antimicrobial selection. Antibiotic challenge is the gold standard diagnostic test. We hypothesized that outpatient or home drug challenge, without prior allergy testing is a safe method of diagnostic testing in most children. METHODS: A retrospective review of drug challenges performed in an outpatient or home setting of children with reported antibiotic allergy. Data entry included antibiotic, allergic reaction, allergy testing, challenge location and outcome. Patients were telephoned one week after the initial challenge to capture any delayed reactions. RESULTS: Antibiotic challenges (43% single dose, remainder 3-5-day course) were studied in 39 patients (59% female) with a history of allergic reactions when aged 6 months to 16 years old (median 3 years). Drug allergy history included a rash only in 75%, and gut symptoms in 8%, or anaphylaxis (with respiratory symptoms or drowsiness) in 15%. Betalactams (penicillin V, amoxicillin, amoxicillin/clavulanic acid, flucloxacillin) were implicated in 95% of patients.
