A 61-year-old man developed pneumonitis and pneumothorax during treatment with atezolizumab for non-small cell lung cancer (NSCLC), and he exhibited lack of efficacy during treatment with prednisone for pneumonitis and pneumothorax [routes, dosages and outcomes not stated]. \* The man, who had history of interstitial lung disease and pulmonary emboli, presented to hospital with shortness of breath and increasing cough. Chest CT scan and positron emission tomography findings were indicative of NSCLC. Biopsy findings confirmed invasive adenocarcinoma. Thus, wedge excision was performed. Subsequently, he received pemetrexed and cisplatin. After chemotherapy, he started receiving atezolizumab. Six weeks after first dose of atezolizumab, he developed a nonproductive cough and shortness of breath. Chest CT scan revealed a new circumscribed ground glass density nodule in the left lower lobe. Atezolizumab-associated pneumothorax and grade III pneumonitis were suspected. Hence, the man's atezolizumab therapy was discontinued.
