Medial Patellofemoral Ligament Reconstruction by Gregory Anderson; David R. Diduch is a Medicine article available to read on EtoBox.

What is Medial Patellofemoral Ligament Reconstruction about?

The medial patellofemoral ligament (MPFL) is known to be the primary soft tissue restraint against lateral translation of the patella during the first 30 of knee flexion. 1,2 After the 30 of flexion, the patella engages the trochlear groove, which provides stability throughout the remainder of the flexion arc. When the patella undergoes a lateral dislocation, the MPFL has been shown to tear at least 87% of the time, leading to the loss of this primary soft tissue stabilizer. Historically, attempts were made to repair the ruptured ligament; however, that practice has now fallen out of favor owing to inconsistency in the results and late failures. 3,4 Reconstruction of the MPFL has largely replaced repair of the ligament. This article focuses on tips and tricks to reconstruct the MPFL both in preoperative planning and intraoperative troubleshooting. An emphasis is placed on graft selection, patellar attachment, and the all-important femoral attachment.
