Multiple drugs Various toxicities, off-label use and lack of efficacy: case report A 61-year-old man exhibited lack of efficacy during off-label treatment with favipiravir, dexamethasone \* and ambroxol for COVID-19, and during treatment with levilimab for cytokine storm. Additionally, he developed Klebsiella pneumoniae pneumonia, Acinetobacter baumannii pneumonia and nosocomial pneumonia during treatment with dexamethasone and levilimab. The man was admitted with a diagnosis of moderate COVID-19 detected using SARS-CoV-2 PCR test, complicated by bilateral pneumonia. His BMI was 30.9 kg/m 2 . Five days prior hospitalization, he developed dry cough, general weakness and malaise. Before admission, he received levofloxacin, favipiravir, amoxicillin/clavulanate and rivaroxaban. Following admission, laboratory tests showed increase in serum lactate dehydrogenase and mild thrombocytopenia. Therefore, off-label IV favipiravir 800mg BID was continued, and nadroparin with off-label dexamethasone and oral ambroxol 30mg TID were added. Low-flow oxygen at flow rate of 5 L/min was initiated.
