What is Chapter 7 Clinical Trials in Epilepsy about?
Almost all forms of seizure disorders should be treated because of the deleterious consequences of recurrent seizures. Approximately 50% of previously untreated patients with epilepsy achieve a permanent remission by the first prescription of a marketed antiepileptic drug (AED), and an additional 10-20% are controlled by dosage adjustment of this drug or alternative monotherapy. Thus, medical treatment remains unsatisfactory for 20-30% of patients with epilepsy. Furthermore, it has been estimated that approximately 30% of patients treated with available AEDs experience moderate to severe adverse effects. 1 Therefore, the need remains for new agents with higher efficacy or lesser toxicity. 2 In 1857, Sir Charles Locock reported a clear reduction in seizures among women given potassium bromide; this simple report resulted in widespread use of this substance. In 1912, Hauptmann reported the antiepileptic properties of phenobarbital prescribed as a sedative. For a long period, showing acceptable therapeutic value was enough. Phenytoin, carbamazepine, and valproic acid, at least in Europe, were approved early after their discovery after open clinical trials only. However, as such trials
