Methotrexate/tofacitinib Vomiting, hyponatremia and lack of efficacy: case report A 68-year-old woman developed hyponatremia and vomiting during treatment with methotrexate for sjogren syndrome. Subsequently, she exhibited lack of efficacy during treatment with tofacitinib for sjogren syndrome [routes, dosages, durations of treatments to reactions onsets not stated; not all outcomes stated]. The woman was presented with pain in multiple joints for 16 years. She received methotrexate for articular flare and positive rheumatoid factor. She developed hyponatremia and vomiting due to methotrexate and was hospitalised. After the correction of hyponatremia, she started receiving tofacitinib. Despite this she experienced grade 2-3 tenderness of most peripheral joints and was bed-ridden (lack of efficacy). Based on investigation, she was diagnosed with sjogren syndrome. Her treatment with tofacitinib was switched to upadacitinib. Thereafter, her condition improved.
