Acquired Craniofacial Features Case 1: A 22-year-old man had taken phenytoin continuously since the age of 7 years. At age 18 years computerized tomography of the brain showed multiple porencephalic defects and a thickened calvarium. He had progression of course facial features with development of a recessed nasal bridge, mild prognathia, prominent, wide and thick nasal tip and lips, acne vulgaris, and generalized facial hypertrichosis. Case 2: A 21-year-old woman who developed infantile spasms at 7 months of age made little developmental progress since the age of 12 years. She received phenytoin from the ages of 12 -13-1 :i years and 15 to 17 years of age. She had spasticity, microcephaly, and contractures.Case 3: Since the age of 10 years, a 29-year-old man had been treated with phenytoin. He currently has spastic quadriparesis, scoliosis, acquired joint contractures, small I testes, pseudobulbar paresis, and one fingertip arch on each hand. ¡ Case 4 : Phenytoin therapy was initiated at the age of 2 years and continued to 18 years of age in a 22-year-old woman for treatment of a nonfebrile seizure disorder. She shows signs of cerebellar dysfunction and borderline microcephaly. I
