What is Pure Autonomic Failure about?
A 1925 report by Bradbury and Eggleston first described patients with extreme orthostatic hypotension and a low, steady heart rate. Evidence accumulated over the next two decades that patients with orthostatic hypotension include those with pure autonomic failure (PAF), characterized by isolated peripheral autonomic dysfunction and decreased norepinephrine synthesis. Over the past decades it has been described that PAF is an α-synucleinopathy, characterized by deposits of α-synuclein, similar to Parkinson disease (PD) and dementia with Lewy bodies (DLB). PAF occurs sporadically, with some cases of PAF actually corresponding to a prodromal synucleinopathy, with these patients eventually developing a central neurodegenerative disorder, such as multiple system atrophy (MSA), PD, and DLB. Treatment of PAF is directed at relieving symptoms with nonpharmacological interventions and with medications producing volume expansion and vasoconstriction.
