What is Lost Receipt Validation Form about?

This lost receipt form is to be attached to an expense report to validate expenses over the minimum receipt limit when an original receipt is unavailable. It requires the transaction date, amount, vendor name, description of expense, explanation for missing receipt, cost center, printed employee name and signature. The form cannot be used for missing hotel, car rental or airline receipts, which require a copy of the original receipt instead.
