This study compared the Gleason scores from needle biopsies to radical prostatectomy specimens in 78 patients. Grading errors were greatest for well-differentiated tumors, with an accuracy of only 23% for Gleason scores of 2-4 on biopsy. Of biopsies with Gleason scores of 5-7, 78% were correctly graded. All biopsies with Gleason scores of 8-10 were accurate. Overall, 50% of biopsies were under-graded compared to the prostatectomy specimen.
