Background: Patients with aortic valve (AV) infective endocarditis (IE) are at high risk for complications including cerebral/systemic embolization, AV destruction with hemodynamic compromise, and death. We present a case of a patient with AV IE complicated by intracranial hemorrhage (ICH), and severe aortic regurgitation. Case: A 49-year-old man with history of IV drug use was brought to the hospital after being found down at home. Brain imaging on arrival showed a large right ICH with intraventricular extension and obstructive hydrocephalus, necessitating emergent hemi-craniectomy (Figure 1). Blood and intraoperative skull flap cultures were positive for MRSA. ## Decision-making: The patient was initiated on appropriate antibiotic therapy. TTE was of poor quality therefore a TEE was performed which showed a flail right coronary cusp, with severe aortic regurgitation, and a large vegetation on the right coronary and noncoronary cusps. The patient was discussed at the multidisciplinary endocarditis meeting and cardiac surgery was delayed given the size of the ICH to allow for neurologic recovery.
