What is Lack of effect of 2 M urea on Jk(a–b–) cells about?
A 23-year-old grava 2, para 2 Philippino woman was found to have anti-Jk3 on a routine prenatal antibody screen. The antibody showed specificity for both Jk(a+b-) and Jk(a-b+) RBCs. The patient's RBCs typed as Jk(a-b-). A 5 percent suspension of the patient's RBCs incubated for 5 minutes in a 2 M urea solution showed no lysis, as demonstrated by persistent opacity blocking the text label behind the tube (above, second tube from right). A saline control containing a 5 percent suspension of the patient's RBCs gave similar results (far right tube). In contrast, 5 percent RBC suspensions of Jk(a+b-), Jk(a-b+), and Jk(a+b+) were lysed in 2 M urea rendering the solution translucent (first three tubes left to right). Heaton and McLoughlin (TRANSFU-SION 1982;22:70-1) first described this phenomenon in a 22-year-old Samoan man with aplastic anemia and Jk(a-b-) RBCs. The discovery was the result of a serendipitous observation made using an automated PLT counter (Autocounter II, Technicon Corporation, Tarrytown, NY). This counter used 2 M urea to lyse RBCs.
