What is Diabetic Retinopathy, a Neurovascular Unit Disease about?
Diabetic retinopathy, a neurovascular unit disease 3 Impairment of diverse cell types, rather than just vascular cells, in the diabetic retina has led to a new paradigm that diabetic retinopathy (DR) should be defined as a disease of the neurovascular unit (NVU). The concept of NVU, which was originally applied to the central nervous system (CNS), has now been utilized to understand retinal circuits and the blooderetinal barrier (BRB). 1e3 The distinct molecular characteristics of NVU depend on the segmentally specific vasculature of CNS. Because of the heterogeneity of cellular components of CNS, no single prototype of NVU is replicated at all levels of the vascular network. However, the concept of any specific NVU module has to be understood that neuronal signals regulate nearby microvessels to support the metabolic needs of the cells of CNS. In this regard, the inner retinal NVU (iNVU) comprises vascular cells, neurons, Mu ̈ller glia, astrocytes, and extracellular matrix (ECM), while the outer retina/choroid neurovascular unit (oNVU), which was previously called neurovascular complex, 5 consists of photoreceptors, Mu ̈ller glia, retinal pigment epithelium (RPE) with its ECM,
