A 15-year-old girl presented with an annular erythematous lesion on the sole of her foot. A few days later, she developed a diffuse papulosquamous rash on her trunk and limbs. Examination and history were consistent with a diagnosis of pityriasis rosea. Notably, the initial solitary lesion on her foot represents an uncommon presentation of a "herald patch", which typically occurs on the thighs, arms or trunk to mark the onset of pityriasis rosea.
