This case report describes an unusual cystic odontogenic tumor in a 16-year-old male, characterized by a well-defined radiolucent lesion with internal radiopaque foci affecting the right mandible. Histopathological examination revealed features resembling both a dentigerous cyst and primordial odontogenic tumor, with multicystic areas and structures akin to dental papilla and enamel organ. The patient underwent excisional biopsy and has shown no evidence of recurrence after 36 months of follow-up.
