This case report describes a 20-year-old male who presented with abdominal pain and was found to have a giant hydronephrosis of the left kidney secondary to ureteropelvic junction obstruction. Imaging revealed a large pelvicaliceal dilation of the left kidney extending into the pelvis. The patient underwent an open left nephrectomy to remove the hydronephrotic kidney.
