## Bilateral herpes simplex keratoconjunctivitis and lack of efficacy: case report A 3-year-old girl developed bilateral herpes simplex keratoconjunctivitis during treatment with dexamethasone as a postoperative treatment. Subsequently, she exhibited lack of efficacy during treatment with cefotaxime, ciprofloxacin and vancomycin for bilateral herpes simplex keratoconjunctivitis [dosages not stated]. The girl had undergone bilateral lateral rectus recessions (limbal incision) for an intermittent distance exotropia (strabismus). The surgery was uneventful and she started receiving topical dexamethasone 0.1% and chloramphenicol drops both 4 times daily for 3 weeks. The post-surgery recover was good in initial week. However, after 8 days of the surgery, she presented with right upper lid swelling, mild ptosis and erythema. She also had mild fever. Over 2 days, her condition worsened with involvement of the left upper lid and severe crusting in both the eyes. She was hospitalised and started treatment with IV cefotaxime 4 times daily, IV vancomycin 3 times daily and topical ciprofloxacin 0.3% 4 times daily. However, she had lack of response.
