What is Validity of Umbilical Artery Blood Velocimetry in the Prediction of Intrauterine Growth Retardation and Fetal Compromise about?

Since MCCALLUM [7] and FITZGERALD & DRUMM [2] published their first results on 'qualitative' ultrasound Doppler shift to estimate blood velocity in the umbilical artery, many centers have followed their example. Relatively low diastolic velocities are seen in intrauterine growth retardation [1,3,4] and therefore this non-invasive method is enthusiastically adopted as a clinically useful method to screen for IUGR. Adequate blood flow through the umbilical circulation is certainly essential to provide the fetus with oxygen and nutrients. Therefore, umbilical cord haemodynamics ought to be of interest in the assessment of fetal growth. The present investigation was undertaken as a part of a prospective longitudinal study to evaluate the predictive value of blood velocity parameters in the umbilical and central fetal circulation (i.e. thoracic and abdominal aorta) in relation to IUGR [10,11]. In this report, the validity expressed as sensitivity, specificity, likelihood ratio and post-test probability of Doppler velocimetry to predict IUGR and fetal compromise is evaluated. ## Patients and methods The study population consisted of 125 unselected pregnant women with a single fetus. The
