An 88-year-old man presented with confusion and right facial droop after being found on the ground. Imaging showed acute bilateral subdural hematomas (SDHs) and subarachnoid hemorrhage. The SDHs were allowed to liquefy before surgical drainage via three bore holes, one on the right and two on the left. The patient experienced post-operative coagulopathy requiring multiple blood transfusions but recovered and was discharged with neurological deficits from his prolonged hospital stay.
