On July 23, 2016, a 60-year-old volunteer firefighter was struck and killed by a tanker backing into the bay of the fire station in Pennsylvania. The firefighter had 15 years of experience but Pennsylvania has no statewide training requirements. The tanker driver could not see the firefighter near the back of the vehicle. The report recommends ensuring safe backing procedures, high-visibility clothing, and cameras or sensors to assist drivers.
