This case report describes a 56-year-old male smoker who presented with persistent dysphonia. Examination revealed granulating leukodystrophy on the left vocal cord, suggestive of neoplasia. Biopsy showed giant-cell granulomatous laryngitis. Syphilis serology was positive, indicating recent infection. Treatment with penicillin resolved the vocal cord lesion and dysphonia. Laryngeal syphilis is rare but can mimic carcinoma. This case highlights an unusual presentation of syphilis confined to the larynx.
