Apixaban Lack of efficacy: case report A 56-year-old man exhibited lack of efficacy during treatment with apixaban as anticoagulant therapy. The man, who had history of lower limb deep vein thrombosis and atrial fibrillation was receiving treatment with oral apixaban 5mg twice daily as anticoagulant therapy. Concomitantly, he received digoxin. Later, he presented to hospital with transient leftsided arm paraesthesia without chest pain. On examination, systolic murmur over the aortic area and splinter haemorrhages in fingernails were noted. He had no significant family history of prothrombotic disorder or history of tobacco smoking, alcohol and recreational drug use. Coronary angiography revealed slow flow in the distal left anterior descending artery suggesting coronary artery embolism. Cerebral magnetic resonance imaging revealed numerous foci of restricted diffusion, including two in the right parietal vertex and one in the right cerebellar hemisphere, consistent with embolic stroke (lack of efficacy).
