Background: Reasons why certain smokers develop chronic obstructive lung disease (COPD) are unknown, but colonization with Pneumocystis jirovecii (Pc) is increased in COPD and associated with increased disease severity. Patients with COPD can be characterized by radiographic and physiologic criteria, and recognition of a specific phenotype related to Pc colonization would improve understanding of the relationship of Pc and COPD and enable identification of at-risk patients for possible future treatment. Objective: To determine if Pc colonization is associated with a specific COPD phenotype either by radiographic or physiologic parameters. Methods: DNA was extracted from 100 non-HIV-infected COPD lung samples obtained from the Lung Tissue Research Consortium (LTRC). Pc colonization status was determined by nested polymerase chain reaction at the mitochondrial large subunit locus. Clinical, physiologic and radiographic data were compared between Pc-positive and negative subjects using Wilcoxon ranksum or Fisher's exact.
