What is 505: TENSION PNEUMOPERITONEUM POST-COLONOSCOPY about?
Superior mesenteric arteriovenous fistula(SMAVF) is an extremely rare vascular disorder and the majority of cases reported have been due to abdominal trauma or gastrointestinal surgery. This fistula was revealed by portal hypertension, with bleeding esophageal varices, ascites, and diarrhea. We report a case of an idiopathic SMAVF without any history of abdominal trauma or gastrointestinal surgery. Methods: A 66-year-old woman with no significant medical history was admitted to our hospital because of bloody massive diarrhea and fatigue from 5 days before administration. Enhanced computed tomography revealed a large aneurysm filled by the superior mesenteric artery(SMA), draining into the superior mesenteric vein(SMV). Celiac artery was occluded at the root and SMA was responsible for almost all blood flow in the abdominal organs. So, we thought portal hypertension due to SMAVF as the cause of diarrhea. Emergency angiography demonstrated SMAVF in accord with CT, and it was treated with stent graft placement and coil embolization. Completion angiography showed minimal flow into the aneurysm. After the procedure, the patient was monitored carefully in the intensive care unit for any
