## Pembrolizumab Type 1 diabetes mellitus and diabetic ketoacidosis: case report A 68-year-old woman developed type 1 diabetes mellitus (DM) and diabetic ketoacidosis following treatment with pembrolizumab for lung adenocarcinoma [route not stated]. The woman was referred to a hospital for chest abnormalities. Chest X-ray and CT showed tumor in the right lower lobe and mediastinal lymph node swelling. Head MRI revealed a metastatic brain tumor. Transbronchial tumor biopsy showed adenocarcinoma and she was clinically diagnosed with right lower lobe Stage IVB lung adenocarcinoma cT4N2M1c. A genetic analysis revealed the primary tumor was positive for the human epidermal growth factor receptor 2 gene. Immunostaining for the expression of programmed death ligand 1 was negative in tumor cells. Based on these results, she started receiving combined immunotherapy with pembrolizumab 200mg along with carboplatin and pemetrexed as first-line therapy. Eventually, she achieved clinical improvements and a partial response. After four courses, maintenance therapy with pembrolizumab 200 mg/body along with pemetrexed was initiated.
