What is Disseminated Infection with Trichosporon beigelii about?
1\vo cases of systemic infection with Trichosporon beigeli! are reported. Both patients had acute leukemia and were receiving cytotoxic and antibiotic drug therapy, which included amphotericin B, at the time of sepsis. Although clinicalisolatesof the organisms were found to be sensitive to amphotericin B in vitro, both patients died from severe, widespread fungal infection. The pathologic findingsin these two casessuggest that the host responseto trichosporon infectionis a granulomatous inflammation. Trichosporon is a virulent opportunistic pathogen that may originate from the gastrointestinal tract damaged by cytotoxic therapy in the patient with aplasia. Despite aggressive antifungal therapy, survival is most closelyrelated to recovery of the host's hematopoietic system. Trichosporon beigelii [1,2] (formerly Trichosporon cutaneum)is an inhabitant of soil and occasionally constitutes part of the normal flora of human skin. The fungus is known to cause the superficial infection known as white piedra. In 1962Gilbert and Fetter showed that large inocula of Trichosporon caused fatal disseminated infections in rabbits [3]. Trichosporon sepsis was first recognized in 1970 [4], but only
