Purpose/Objective(s): The tolerability and efficacy of hypofractionated radiotherapy has been demonstrated in the treatment of pelvic malignancies. Studies have shown the feasibility of IMRT in treatment of prostate, gastrointestinal tract, and gynecologic cancers. There are none reporting hypofractionated intensity-modulated radiotherapy (HF-IMRT) to the pelvis for colorectal or anal cancer. We report the toxicity with pelvic ± inguinal HF-IMRT compared to a historical control of standard fractionation (SF) reported by RTOG 98-11 for anal canal cancer. Materials/Methods: Between 3/03 and 1/07, eighteen patients received 5FU-based chemotherapy and concomitant pelvic ± inguinal HF-IMRT for primary or recurrent colorectal or anal canal malignancies. Simultaneous integrated boost technique was used: primary tumor and immediately adjacent lymphatics: 225 cGy  22 fx, lower pelvic lymph nodes: 200 cGy  22 fx, and upper pelvic lymph nodes: 160 cGy  22 fx. Toxicity was scored utilizing CTCAE2.0 criteria and RTOG Late Radiation Morbidity Scoring Schema and compared to RTOG98-11 (n = 627) toxicity profiles.
