The ensuing oxidative stress and tissue remodeling compromises the integrity of the delicate LC/OHN and retrograde depletion of brain-derived neurotrophins and mitochondria kills the RGCs. Eventually when thousands of RGCs have died, the patient becomes aware of visual impairment that steadily progresses over many decades and culminates in blindness in the absence of any treatment. ## Primary open-angle glaucoma and its treatment Many epidemiological, morphological, and pathological investigations have concluded that advanced age, elevated IOP, family history of glaucoma, black race, low intracranial fluid pressure, poor retinal blood flow (causing ischemia), IOP fluctuations, and perhaps diabetes are responsible for causing POAG. Of all these culprits, high IOP is the only modifiable risk factor that can be treated at this time. Since elevated IOP (or ocular hypertension, OHT) results from the inability of the AQH to leave the ANC, all current treatments for POAG are focused on lowering and controlling IOP.
