What is (449) An Overlooked Contributor to CAV about?

Purpose Triglycerides (TG) are involved in CAD and metabolic syndrome and interact negatively with LDL and HDL. Statins have slight impact on TG levels but mainly exert their effect on lowering LDL and increasing HDL. Statins are included in ISHLT guidelines for care of the heart transplant recipient to prevent CAV. However, the relationship between TG and incidence of CAV is unknown. Methods We analyzed a modern cohort of heart transplant recipients in our center and divided them into patients with CAV and patients without CAV to assess their TG and LDL levels. Our analysis consisted of 230 patients who were followed for at least one year after OHT, with the population being 30% female, 30% ischemic cardiomyopathy as indication for transplant. Results Over follow-up of 1263 patient years 17% of patients developed CAV at a median of 5.1 years s/p transplant. At CAV diagnosis 50% of patients were on a high potency statin, while 18% were on an omega-3 agent. There was no difference in LDL levels at CAV diagnosis nor 1 year prior to diagnosis compared to those without CAV. Patients who developed CAV had significantly higher TG levels at CAV diagnosis (p=0.008) and higher levels 1 year
