Refeeding syndrome occurs when severe fluid and electrolyte shifts happen in malnourished patients undergoing refeeding as insulin levels increase, driving potassium, phosphate and magnesium into cells. This can lead to organ dysfunction and even death. Patients at high risk include those with a BMI below 16 kg/m2 or unintended weight loss over 15% in the last 3-6 months. For these patients, feeding should start at just 10 kcal/kg daily and increase slowly, with potassium, magnesium and phosphate supplement
