The OPTIMUM randomized clinical trial compared microultrasonography-guided biopsy with MRI fusion-guided biopsy for prostate cancer diagnosis in biopsy-naive men. Results showed that microultrasonography-guided biopsy was noninferior to MRI fusion-guided biopsy for detecting Gleason Grade Group 2 or higher cancers, with detection rates of 47.1% and 42.6%, respectively. The study suggests that microultrasonography may serve as a viable alternative to MRI for image-guided prostate biopsy.
