The collision occurred between the Panama-registered container ship Leo Perdana and the Indonesian-registered gas tanker Navigator Aries in the Surabaya Western Access Channel on June 28, 2015 at around 23:17 local time. Both ships had agreed to a port-to-port passing but the Leo Perdana experienced a malfunction with its rudder, causing it to drift into the path of the oncoming Navigator Aries.
