A fatal incident occurred during a rescue boat drill on a car carrier operated by NYK Line in the South Atlantic Ocean on August 9, 2011. During the drill, the chief officer and a crew member boarded the rescue boat, which was swung out from the No. 11 deck. A few seconds later, the rescue boat suddenly fell into the sea. The chief officer unfortunately died after being transported to the closest port, while the crew member was hospitalized. An investigation found the safety pin was in an incorrect position
