Elemene is a small-molecule, anticancer, active ingredient based on the principle of “molecular compounds” of integrated traditional Chinese medicine (TCM) and Western medicine. Clinical studies have shown that elemene has various cancer-suppressive effects, enhances autoimmune function, and shows significant advantages in improving patient quality of life, prolonging overall patient survival, inhibiting metastasis and recurrence, and reversing multidrug resistance. Moreover, it shows a short half-life and induces severe phlebitis and other side effects after intravenous injection. Therefore, it is essential to explore new drug delivery systems and new combination therapies combining TCM and Western medicine of elemene. This chapter focuses on the latest research results related to the novel elemene drug delivery system, combination therapy strategies using elemene, and combination therapy using elemene and mRNA drugs. These findings may bring new opportunities for elemene to be developed as an effective anticancer drug.
