S Tacrolimus Various toxicities due to supratherapeutic levels: case report A 67-year-old man had cardiac arrest, torsades de pointes (TdP) and QT prolongation due to supratherapeutic level during immunosuppressive treatment with tacrolimus [route, dosage and outcomes not stated].The man, who had a history of familial nephritis requiring bilateral renal transplant, presented to the emergency department following cardiac arrest. He had been receiving immunosuppressive treatment with tacrolimus.The man was found by EMS, and cardiopulmonary resuscitation (CPR) was initiated. He was defibrillated a total of 8 times. After his second resuscitation, he developed TdP. Hence, he was quickly given magnesium. The electrocardiogram (EKG) showed ST elevations, while coronary angiography showed no occlusion. It was reported that, one week before admission, he was found to have supratherapeutic levels of tacrolimus. The heart MRI showed normal findings, and repeat EKG revealed sinus rhythm with a QTc of 548ms.
