What is Profile of Urticaria in an Inner-city Pediatric Emergency Department (ED) and its Correlation with Food Allergy about?
RATIONALE: Guidelines recommend H1-antihistamines as the first-line therapy for chronic urticaria (CU). However, refractory CU not responding to standard dose of H1-antihistamines remains unclear. METHODS: This retrospective cohort study included adults with CU who visited a single tertiary hospital in Korea from January 2002 to December 2020. Patients treated with more than three H1-antihistamines or omalizumab were defined as refractory CU. RESULTS: 3,141 CU patients were enrolled, of which 38.2% were refractory CUs. Refractory CU patients showed higher rate of follow-up period over a year (34.0% vs 20.9%, P < 0.001), combined cholinergic (2.8% vs 1.5%, P 5 0.017), and pressure urticaria (2.9% vs 1.7%, P 5 0.031), than non-refractory CUs. Other control medications were more frequently used in refractory CUs: sedative H1-antihistamines (99.6% vs 81.9%), leukotriene receptor antagonists (48.6 vs 26.0%), cyclosporine (4.8% vs 1.6%), corticosteroid (46.7% vs 36.4%; all P < 0.001), and omalizumab (11.4% vs 0%). They also showed higher serum WBC count (6730 vs 6470/uL, P 5 0.002), neutrophil count (3900 vs 3690/uL, P 5 0.001), aPTT (27.3 vs 26.3 sec, P < 0.001), C3 (108.9 vs 105.4 mg/d
